Startup & Licensing
Get licensed without guessing what the state wants.
Most first applications stall on small things: a missing policy, an org chart that doesn't match the staffing plan, a clinical director who isn't quite qualified on paper. We build the file the way the surveyor reads it.
What it is
From entity to approved license.
Licensing is paperwork with teeth. California reviewers don't grade effort — they grade completeness and consistency. We assemble the entire application package, write the policies and procedures to match your model, and stay on the file through every request for additional information until you're approved.
What's included
Everything the file needs — assembled, not outlined.
No generic binder. Each piece is built to your model and cross-checked against the others, so a reviewer never finds the seam.
Entity & enrollment
Formation, EIN, NPI types 1 and 2, and Medicare/Medi-Cal enrollment set up correctly the first time, so a clerical mismatch doesn't reset your clock.
Policies & procedures
A full P&P manual written to your actual service lines and patient population — not a generic template a reviewer has seen a hundred times.
Application package
Forms, org chart, staffing plan, job descriptions, governing-body documents, and disclosures assembled so they agree with one another line for line.
Survey-ready operations
QAPI program, clinical records system, and required logs in place before the initial survey — not scrambled together the week before.
Reviewer correspondence
We answer the state's requests for additional information directly, in their language, and keep the file moving.
The GTR process
Four steps, carried by us — not handed back to you.
We confirm the path before a dollar is spent, build the file, shepherd it through review, and stand with you on survey day.
Model & jurisdiction
We confirm the right license type, agency, and pathway for what you actually intend to operate — before a dollar is spent on the wrong application.
Build the file
Entity, enrollments, P&P manual, and the full application assembled and cross-checked for internal consistency.
Submit & shepherd
We file, track, and respond to every reviewer request until the application clears review.
Survey readiness
We prepare your team and your records for the initial licensing survey and stand with you through it.
What California requires
The standards the state and accreditors actually measure you against.
DHCS, CDPH, and the accrediting bodies don't grade effort — they grade completeness and consistency. Here's what has to be true on paper.
DHCS / CDPH licensure
Depending on your model, your license runs through the Department of Health Care Services or the California Department of Public Health. The path, fees, and survey differ — we map yours up front.
Approved policies & procedures
California expects P&Ps that match your service lines, staffing, and patient population, with version control and governing-body adoption.
Qualified administrator & clinical lead
Most license types require a named administrator and a clinical supervisor who meet specific experience and credentialing thresholds. We confirm yours qualify on paper.
Physical site & zoning
Facility-based licenses require a compliant, zoned, inspected location. We help you avoid signing a lease the state will later reject.
We'd been told the license would take three months and were nine months in with two other consultants. GTR rebuilt the file, and we were approved the next cycle.

FAQ
Questions founders ask us
How long does licensing take in California?
It depends on the license type and how clean your file is. A well-prepared home health application typically moves faster than a facility-based detox license, which involves site inspection. The biggest variable isn't the state — it's how many times your file gets sent back. We build it to not come back.
Can you take over a stalled application?
Often, yes. We review what's been filed, find why it stalled, and decide whether to repair the existing file or restart cleanly. We'll tell you honestly which is faster.
Do you guarantee approval?
No one credible guarantees a government approval, and you should be wary of anyone who does. What we control is the quality and completeness of the file, and how quickly we respond to the state. That's where applications are won or lost.
